S254 Community and Social Care
This module introduces students to the unique needs of seniors and the wide range of community health and social resources available to support ageing-in-place.
Students will examine ageing factors, developmental theories, common health conditions, care needs, cognitive and functional assessments, caregiver burden, and Singapore’s integrated care model. They will apply communication and engagement frameworks to build rapport, support care conversations, and recommend relevant community programmes and resources. Through Preventive Health Visit (PHV) training and authentic senior engagement, students will practise the B.E.L.I.E.F. approach, articulate the role of Silver Generation Ambassadors, administer the Senior Engagement Form (SEF), interpret collected information, and communicate SG initiatives to seniors as part of SGA certification requirements.
