S352 Case Management
This module introduces students to case management within Singapore's community setting.
Case management is a collaborative process encompassing assessment, planning, implementation, coordination, monitoring, and evaluation of client and caregiver needs, alongside recommending appropriate community services. Students will gain insights into the complexities of coordinating care among stakeholders, service providers, clients, and caregivers, aiming for optimal health outcomes and cost-effectiveness.
