This module introduces students to the role of integrated care in the Singapore Healthcare System.

Students will explore the role of a sustainable integrated care framework in bolstering the efficacy of healthcare services. They will delve into the intricacies of the healthcare system (Primary, Acute, and Community Care sectors) and the funding mechanism. Students will learn about how coordinated transition across these sectors enhances the continuity of holistic care. This module will emphasise the pivotal role played by the Community Care sector.
