To better meet the evolving healthcare needs of Singapore's greying population, the Specialist Diploma in Integrated Care Management prepares participants for the role of a Care Coordinator Associate (CCA) as an alternative career progression pathway within the healthcare sector.
Graduates will be equipped with the skills and know-how to collaborate with a multidisciplinary team of medical professionals in the delivery of transitional care in public, private and Intermediate and Long-Term Care (ILTC) health institutions.
Through practical, industry-focused learning, you will be able to:
Take on the role of a Care Coordinator Associate
Gain the skills and know-how to support medical professionals in the delivery of transitional care

Leadership and Outcome Management | This module focuses on the management of wards, units, hospitals, community and other healthcare organisations using principles of effective leadership and management. It prepares you to make decisions, minimise and manage risks to ensure patient safety within the healthcare environment. The module also focuses on interdisciplinary collaboration in improving patient outcomes. You will appreciate the activities that contribute towards the improvement of the quality of patient care and support hospital activities/organisational goals. You will gain knowledge in the optimal utilisation of resources, efficiency in the delivery of high-quality care, patient flow, capacity management and other clinical cost reduction initiatives. |
Advanced Care Coordination and Navigation | This module provides you with an overview of the dynamics of advanced care integration and how it achieves the best care outcome for clients. You will gain an appreciation of various financial schemes and community resources available for the patients, families and caregivers. You will develop plans to coordinate with the multidisciplinary team of health professionals, such as physical therapists and occupational therapists, in the promotion of home rehabilitation. |
Project on Social and Family Dynamics | This student-led project focuses on family dynamics across the lifespan with an emphasis on the elderly. The module will allow you to understand interdisciplinary collaboration, innovative action and the well-being of individuals, families and social institutions through active community involvement. |

Integrated Care and the Singapore Health System | Complex older adults, such as those with hip fractures, frequently require care from multiple professionals across a variety of settings. Integrated care between providers and across settings is important to ensure care quality and patient safety. This module focuses on the application of the principles of integrated transitional care in the health system. It aims to equip you with the knowledge of patient assessment for biopsychosocial needs, nursing and financial needs in both inpatient and outpatient settings. |
Advanced Integrated Care Planning | Integrated transitional care helps to plan healthcare along systems and disciplines in multiple environments with a goal of making healthcare users’ experience personalised, flexible and seamless. This module aims to equip you with the knowledge and skills to develop comprehensive care plans to effectively utilise community resources for care coordination and co-manage patients with chronic diseases in the community. It also introduces students to the principles of palliative care for advanced care planning. |
Project on Communication and Health Coaching | The student-led project equips students with advanced communication and health coaching skills. You will be required to apply theoretical frameworks and evidence-based approaches such as motivational interviewing. The focus will be on the health behaviour change strategies used for clients to manage chronic diseases and mental health. |
Entry requirements
WSP Eligibility
Singaporeans and Singapore Permanent Residents who are:
within three years of graduation from the Polytechnics or the Operational Ready Date (ORD) for full-time National Servicemen, or
within five years of graduation from the Polytechnics, and if their employer is a participating company.

Admission will be reviewed and approved by the polytechnic on a case-by-case basis.
