This module introduces the students to community case management (CCMS). CCMS is a dynamic, collaborative process designed to cater to the needs of clients/ seniors living in the community and their caregivers, together with the support of the various community care providers. Throughout this module, students will be exposed to case management tools such as community screening, interrail assessment, individualized care planning, home assessment and resources management (Right siting of care). Other topics include crisis and conflict management and resolution. Students will gain an appreciation of the complexities in the coordination of care amongst various stakeholders such as the service providers, clients and their caregivers while maintaining the desired health outcome and cost.
