Care for Seniors with Chronic Illnesses

Chronic disease is a major health concern which may be slow in progression and long in duration. It affects the productivity and quality of the life of an individual. This module focuses on the care for seniors with chronic diseases in the community.

It introduces students to the knowledge that addresses elderly’s self-management and accessibility to community information and resources, encourages elderly to engage in active living and ageing in place, promotes appropriate health behaviour and autonomous decision-making, and advocates advance care planning of chronic diseases.   

Care for Seniors with Chronic Illnesses is a module selected from the Specialist Diploma in Healthy Ageing & Community Care for Seniors.  It enables participants to acquire new skills or deepen relevant skills without the need to pursue a full specialist diploma. As this is a stackable course, participants may choose to complete the remaining modules within the validity period, of 2 years, to be awarded the full Specialist Diploma qualification.

How often do I need to attend the course?

Lessons will be held at Republic Polytechnic @ Woodlands

Course commences in April.

Number of Lessons Duration 
Three times/week (weekdays) 7pm to 10pm
