Multi-disciplinary and Multi-sectoral Care for Seniors (including case management)

To promote healthy ageing and caring for seniors in the community involve the delivery of health and social care across various disciplines and sectors. This module aims to equip graduates with knowledge, skills, attitudes and competencies for effective case management. Specifically, graduates will be taught on how to coordinate care across care continuum and sectors with the aim of assisting clients and family members to navigate the complex healthcare system. One of their major role is to collaborate and liaise with community partners for client’s continuity of care in the community.

They will also be taught on skills to leverage on community partners & resources for follow up and support e.g. finance, transport, meals, escort services, medical equipment, appointments etc. Graduates will be taught on the importance of maintaining effective communication and interpersonal relations.   

Multi-disciplinary and Multi-sectoral Care for Seniors (including case management) is a module selected from the Specialist Diploma in Healthy Ageing & Community Care for Seniors.  It enables participants to acquire new skills or deepen relevant skills without the need to pursue a full specialist diploma. As this is a stackable course, participants may choose to complete the remaining modules within the validity period, of 2 years, to be awarded the full Specialist Diploma qualification.

How often do I need to attend the course?

Lessons will be held at Republic Polytechnic @ Woodlands

Course commences in April.

Number of Lessons Duration 
Three times/week (weekdays) 7pm to 10pm
