This module provides an overview of Community Care in Singapore with a focus on its evolution, key principles, types of services available and how it supports the ageing population. Students will be exposed to Care “by” the Community which refers to the continuum of integrative care for people who require further care and treatment after being discharged from a general hospital, as well as care for individuals to age in place. It introduces the resources, funding and subsidies available, as well as ethical and legal considerations in Community Care. The module also explores cultural sensitivity, communications, and care planning in Community Care settings. It concludes with a discussion of future trends and challenges in Community Care.
