This module introduces students to the management of chronic conditions across the 11 systems. The students will gain a deeper understanding of the chronic disease model and how to apply it in the management of chronic conditions. Topics include the pathophysiology, etiology, predisposing factors, common diagnostic tests, nursing, medical and pharmacological management of chronic conditions. Students will gain an understanding of the importance of empowering patients to self-manage chronic conditions. Students will learn the self-management strategies and available support for the patients and carers to prevent and manage chronic conditions, particularly for seniors living in the community.
