Integrated Care and the Health System

This module focuses on the application of the principles of integrated transitional care in the health system. It equips participants with the knowledge on patient assessment for biopsychosocial needs, nursing, and financial needs in both inpatient and outpatient settings. It highlights the importance of integrated care between providers and across settings to ensure care quality and patient safety.  

Integrated Care and the Health System is a module selected from the Specialist Diploma in Integrated Care Management.  It enables participants to acquire new skills or deepen relevant skills without the need to pursue a full specialist diploma. As this is a stackable course, participants may choose to complete the remaining modules within the validity period, of 2 years, to be awarded the full Specialist Diploma qualification.

How often do I need to attend the course?


Lessons will be held at RP ACE @ Woodlands. 
 
Course commences in October. Application Start and End date: 17 July 2023 to 31 August 2023.

Number of Lessons Duration 
Three times/week (weekdays) 7pm to 10pm

Note : Some lessons will be conducted via e-learning.
